Does the form verify the client's age?
It records the date of birth the client attests to, with their signature and a timestamp. Physical ID still gets checked at the desk; the form's job is documenting that the check happened and what was attested, which is the evidence that matters afterward.
Why capture health disclosures in the consent?
Allergies, skin conditions, blood thinners, and pregnancy all affect whether and how you work. Collecting them in the signed record proves the client disclosed (or didn't) before the procedure, which protects the artist if healing goes wrong.
Can clients sign before they come in?
Yes, send the link with the appointment confirmation so the paperwork's done before they sit down. A QR code at the counter covers walk-ins; either way the consent is on file before work starts.
