↗ · What it's for

Boarding someone's dog means accepting a living animal with health quirks, behavior history, and an owner three time zones away. This waiver captures what you need before they go: vaccination attestation, behavioral disclosures, authorization to seek vet care (and who pays), and the liability release, each one signed, timestamped, and tied to the right owner and pet.

Worth knowing · A waiver template is a starting point, not legal advice. Liability standards for animal care vary by jurisdiction, so have your release language checked for where you operate.

When to use it

  • Boarding kennels and catteries signing owners in at drop-off
  • Doggy daycares collecting one waiver per dog per season
  • Groomers documenting condition disclosures before working on a pet
  • Pet sitters and walkers getting vet authorization before the first booking

· Questions

The template, answered.

Why include a vet-care authorization?

If a boarded pet needs emergency treatment and the owner is unreachable, you need documented authority to act and clarity on who pays. A checked, signed authorization with a timestamp answers both, which is far better than a phone call you can't prove happened.

Can owners attach vaccination records?

Yes, add a file-upload field (paid plans) and owners photograph the vaccine certificate at signup. Otherwise the attestation checkbox records that they confirmed current vaccinations, with the date attached.

One waiver per visit or per season?

Most facilities collect one signed waiver per pet per year and re-confirm vaccinations on a schedule. Since the form is a reusable link, either rhythm works; the dashboard shows the signing date for every record, so expiry checks are a sort, not a search.

Legal-grade on every plan.

Every signature carries the signer's IP, a UTC timestamp, the user-agent, a SHA-256 hash, and a frozen copy of the form, baked into a signed PDF. Included at every tier, not sold back to you as an upgrade.
